New Patient Information
We welcome new patients to come see one of our skilled physicians. We participate in most insurance plans and networks including United Healthcare, Cigna, Blue Cross Blue Shield, Aetna, and Medicare. Please contact your insurance carrier to ensure participation. Please review your insurance policy prior to scheduling your appointment to verify if a referral from a Primary Care Physician is needed. If so, please call our office before your appointment to ensure that the referral has been received.
Appointment Cancellation Information
If an appointment has to be canceled, please inform our office at least 24 hours in advance. When an emergency or illness arises, we will do our best to reschedule your visit as soon as possible.
Waiting Time Information
We strive to ensure that each patient is seen as close to their appointment time as possible. Occasionally, emergencies do arise. If this is the case, you will be informed promptly of any delay to your appointment time. In order to keep this to a minimum, we ask that patients come to their appointment on time and bring all requested materials with them.
Materials to Bring to your Appointment
Please remember to bring any X-rays, MRI's, CT Scan's or EMG reports that pertain to your injury as well as a list of current medication and dosage information. Please have any other relevant office notes or operative reports from another physician's office sent to us before your scheduled appointment. This allows our physicians to have any pertinent information to review before your appointment. We also request that you bring a form of picture identification with you. Also, please remember to bring your insurance card with you to your appointment. If any of your insurance information has changed, please call our office to update it. Some insurance policies require a co-pay for your visit. This is to be paid at the time of service. We accept cash, checks, money orders and credit cards (Visa, Mastercard, American Express and Discover).
